Consent to the Processing of Personal Data
I (whose personal data is provided in the registration form above) (hereinafter referred to as the “Data Subject”), in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as the “Regulation”) and in accordance with the Act on the Processing of Personal Data No. 110/2019 Coll. (hereinafter referred to as the “Act on the Processing of Personal Data”), as amended, hereby grant the consents listed below, which are independent of one another and may be granted separately.
A. Consent to Receive Invitations and Marketing Materials
I hereby grant my express consent to the German-Czech Chamber of Commerce and Industry, with its registered office at Václavské náměstí 795/40, Nové Město, 110 00 Prague 1, Company ID No.: 497 08 210, registered in the Commercial Register maintained by the Municipal Court in Prague under file no. A 8407 (hereinafter referred to as the “Chamber”), as the data controller, to process my personal data, including my first name, last name, email address, phone number, company name, and position within the company (hereinafter referred to as “Personal Data”), for the purpose of sending invitations, newsletters, business offers, and similar communications from the Chamber, as well as information about other events organized by the Chamber, via email or other communication channels.
Furthermore, I hereby grant the Chamber my express consent to transfer the aforementioned Personal Data to the recipient, Ernst & Young, s.r.o., with its registered office at Na Florenci 2116/15, 110 00 Prague 1, Company ID No.: 267 05 338, registered in the Commercial Register maintained by the Municipal Court in Prague under file no. C 108716 (hereinafter “EY”), which will process this Personal Data as an independent controller for the purpose of:
sending invitations, newsletters, commercial offers, and similar communications from EY, as well as information about events organized or co-organized by EY, via email or other communication channels; and

2. potentially contacting me in the future regarding recruitment by EY.
I grant my consent under this Section A for a period of 3 years from the date of grant or until I revoke it, whichever occurs first.
I acknowledge that this consent is entirely voluntary and is not a condition for registration and participation in the seminar “Critical Infrastructure, Resilience, and New Obligations Across the Supply Chain” to be held on November 9, 2026 (hereinafter referred to as the “Event”); The provision of Personal Data for the purposes of organizing the Event itself and any sharing of such data between the Chamber and EY for these purposes is governed by a separate privacy statement pertaining to the organization of the Event, which is available on the registration form.
I may withdraw my consent under this Section A at any time, with future effect, by sending a simple written notice to the Chamber’s address or electronically to jakl@dtihk.cz, or, with respect to processing by EY, to ivana.svecova1@cz.ey.com. The withdrawal of consent does not affect the lawfulness of any transfer or processing carried out prior to such withdrawal. If I withdraw my consent with respect to the Chamber, the Chamber shall inform EY of this without undue delay.
B. Consent to the Taking and Publication of Photographs
I further grant my consent for the Chamber, or EY as applicable, to take photographs of me during the Event and to further process these photographs for their marketing purposes in connection with the Event, in particular to publish them on their websites (including http://tschechien.ahk.de/ and www.ey.com/cs_cz), on their Facebook and LinkedIn profiles, and in their other publications (e.g., in the magazine *Plus* and the Chamber’s Newsletter), and to modify them for this purpose at their discretion, to incorporate them in whole or in part into a collective work, or to use them in any other way for this purpose.
I acknowledge that the photographs taken by the Chamber or EY, as applicable, in accordance with this consent, will be processed automatically and by employees of the Chamber or EY, respectively, for a period of 3 years following the Event or until I revoke my consent to their processing. I do not request any financial or other compensation from the Chamber or EY for the use of my photograph. Any revocation of my consent will not affect processing that took place prior to the revocation, particularly with regard to photographs published in print.
I may revoke my consent under this Section B at any time, with future effect, by sending a simple written notice to the Chamber’s address or electronically to jakl@dtihk.cz, or, in relation to processing by EY, to ivana.svecova1@cz.ey.com.
C. General Provisions
I provide my personal data voluntarily. I am under no obligation to provide my personal data. Failure to provide consent is not a condition for participation in the Event.
The consents under Sections A and B are independent of one another. Withdrawal of one consent does not affect the validity of the other.
I declare that the Personal Data I have provided is accurate and truthful. Personal Data will not be used for automated individual decision-making or profiling that would have legal effects on me or significantly affect me in a similar manner. I have been informed and instructed regarding my rights under Articles 13–22 of the Regulation.
